The provisionally qualified candidates for the post of Post Graduate Teachers for Telugu and Urdu Subjects vide Notification No. 13/2017 for which the Main Examination was held on 27.08.2017 are hereby directed to attend for the Verification of Certificates on 26.09.2017 at Polytechnic College, Masab Tank, Hyderabad as per their schedule and produce the following original documents along with two sets of Photo-stat copies attested by a Gazetted Officer along with two sets of Basic information data (Check-list) and attestation forms for verification of certificates without fail. Extension of time for producing certificates will not be entertained for any reason.

MEMO FORM(CERTIFICATE FOR VERIFICATION:
1-Date of Birth (SSC or its equivalent)
2-Proof of age relaxation in case of Telangana State Government Servant (Regular Service certificates from the concerned department, Government of Telangana State / NCC instructor certificate/ Retrenched Census service certificate
3-Integrated Community Certificate (issued by the concerned M.R.O./ R.D.O of T.S Government
4-Non-Creamy Layer certificate for BC Community Candidates with father/mother name as per Memo No. 3009/BCW/OP/2011, Govt., of Telangana, BC Welfare (OP) Department, Dt: 18-12-2015.(Prescribed format hosted on Website)
5-Disability Certificate (SADAREM Certificate) in case of PH candidates. The PH candidates have to under go a medical examination at respective Medical Board.
6-School Study Certificate from IV to X Class if studied in School; OR Residence Certificate issued by the MRO in case of Private study for seven years including and prior to SSC.
7-Original Qualification Certificates.
8-Proof of Ex-servicemen, if any
9-Un-Employee declaration for claiming fee exemption for the applicants who are in the age group of 18-44 years (OC Un Employees)
10-NOC from employer for In service candidates
11-Hall Ticket of the Examination held on 27/08/2017.
12-Two latest Passport Size photographs. (Check List)
13-Basic information data (2 copies) in duplicate duly filled in by the candidate ( download from the Commission’s website)
14-Attestation Forms in duplicate (2 copies) duly filled in by the candidate and duly attested by any Gazetted Officer. (Download from the Commission’s website).
He / She is further informed that, failure to comply with the above directions may lead to cancellation of his / her candidature.
